CAUTION: This calculator should NOT be used to determine doses when converting a patient from one opioid to another. This is especially important for fentanyl and methadone conversions. Equianalgesic dose ratios are only approximations and do not account for genetic factors, incomplete cross-tolerance, and pharmacokinetics.
*Buprenorphine has partial µ-opioid receptor agonist activity with resultant ceiling effects compared with full µ-opioid receptor agonists. Buprenorphine transdermal is NOT approved for opioid use disorder, and its inclusion in this calculator should not be applied to clinical decisions related to the management of opioid use disorder.
